McMINNVILLE, Ore. (KOIN) — A 65-year-old woman died when her Toyota was hit head-on by a Ford Flex on Highway 99W Tuesday afternoon in a 3-car crash.

Investigators with the Oregon State Police said the Flex, driven by Terrence Chitwood, crossed the center line and slammed into the Toyota. Driver Kristyanne B. DeVries was first rushed to the Willamette Valley Medical Center and then transferred to OHSU. The 65-year-old Amity woman later died from her injuries.

A Subaru was the third car involved in the crash, officials said. The car was behind the Toyota and wasn’t able to stop after the crash.

The 2 people in the Subaru, both from Corvallis, suffered minor injuries.

Chitwood, 67, and his 66-year-old passenger Doris Chitwood, were both taken to Willamette Valley Medical Center for treatment. Officials said they each sustained minor injuries.

The investigation continues into the cause of the crash.
